GLP-1 medications such as Wegovy (semaglutide) and Zepbound (tirzepatide) have become important treatment options for obesity and certain obesity-related health conditions. However, Medicare has generally not covered these GLP-1 medications when prescribed solely for weight management.
To help address this gap, the Centers for Medicare & Medicaid Services (CMS) launched the Medicare GLP-1 Bridge Program on July 1, 2026.
If you're enrolled in Medicare and wondering whether you qualify for this new program, here's what you need to know.
What Is the Medicare GLP-1 Bridge Program?
The Medicare GLP-1 Bridge is a temporary nationwide demonstration program created by CMS that provides eligible Medicare beneficiaries with access to certain GLP-1 medications that aren't otherwise covered by Medicare Part D.
Unlike your regular Medicare prescription drug benefit, the Bridge Program operates outside of Medicare Part D. CMS administers the program directly through a centralized system that manages prior authorizations, pharmacy claims, and payments.
The program began on July 1, 2026, and is currently scheduled to end on December 31, 2027, unless CMS extends or replaces it.
Who Can Qualify?
To qualify, you must:
✔ Be age 18 or older
✔ Be enrolled in Medicare prescription drug coverage (Part D or a Medicare Advantage plan with drug coverage)
✔ Meet the program's medical eligibility requirements
What Are the Medical Requirements?
You may qualify if one of these applies to you.
Option 1
- BMI of 35 or higher
Option 2
- BMI of 30 or higher
AND
At least one of these conditions:
- Heart failure with preserved ejection fraction (HFpEF)
- Uncontrolled high blood pressure
- Chronic kidney disease (Stage 3a or higher)
Option 3
- BMI of 27 or higher
AND
One of these conditions:
- Prediabetes
- Previous heart attack
- Previous stroke
- Peripheral artery disease with symptoms
Who Doesn't Qualify?
If you're taking a GLP-1 medication for one of these conditions, your medication may still be covered under your Medicare Part D plan, it just wouldn't qualify for coverage through the Bridge Program.
- Your Medicare Part D plan already covers your GLP-1 medication.
- You're taking a GLP-1 medication to treat type 2 diabetes.
- You're taking it for moderate-to-severe obstructive sleep apnea.
- You're taking it for fatty liver disease (MASLD/MASH).
Remember, these conditions may still be covered through your Medicare Part D plan, they just aren't covered through the Bridge Program.
Which Medications Are Covered?
CMS currently covers certain GLP-1 medications through the Bridge Program, including select formulations of medications such as Wegovy and Zepbound.
Because the list of covered medications may change, we recommend checking the official CMS website for the most current information rather than relying on older articles or social media posts.
How Much Does It Cost?
If you're approved, your cost is simple. $50 per month.
A few things to know:
- Your Medicare Part D deductible doesn't apply.
- The $50 doesn't count toward your Medicare Part D annual out-of-pocket limit.
- Extra Help (Low-Income Subsidy) doesn't reduce the $50 payment.
- The Medicare Prescription Payment Plan doesn't apply.
What Does My Doctor Need to Do?
Your healthcare provider must:
- Prescribe a covered GLP-1 medication.
- Complete any required prior authorization.
- Confirm that you're using the medication as part of a treatment plan that includes diet and exercise.
Your provider may also need to submit medical documentation to demonstrate that you meet the program's eligibility requirements.
